The Foundation does not use a standard proposal format. Each request for support should include the following:
Contact information including agency name, contact person name and title, mailing address, phone number, email address and website address if applicable.
A brief description of the history and mission of the agency.
A description of the project, including statement of need, timeframe for completion and how it supports the mission of the agency.
Project budget that includes a detailed revenue and expense plan, committed sources of support, in-kind contributions and other pending grant applications. What specific amount is requested from the Foundation and how will the funds be used?
A detailed estimate of post-completion operating costs and a plan to sustain these additional expenses.
A current agency operating budget, including planned and actual revenue and expenses.
The most recent audit or financial review and IRS 990.
